Appointments

  1. Office hours are Monday, Tuesday and Friday, 9 am to 5 pm.
  2. Appointments can be scheduled either by phone or email. Please contact the office at 413-822-2096 or patients@ardisfisch.com to set up an appointment.
  3. Appointments may be canceled without penalty up to three business days prior to the scheduled time of the appointment during business hours. (Wednesday for Monday appointments, Thursday for Tuesday , Tuesday for Friday ). If sufficient notice is not given, the full amount of the scheduled visit will be charged.
  4. Initial appointments are usually held in person at the office. After the initial appointment, further consultations may be done by phone. The cost of the visit is the same whether done by phone or in person. If you are unable to travel, please contact the office to discuss your concerns and make special arrangements.
  5. After you schedule your initial appointment, please download the “New Patient Packet” from the download section. If you are unable to download it, please ask for a copy to be sent to you via email or snail mail. Please fill out this paperwork and return it to the office at least 3 days prior to your appointment, to allow Dr. Fisch time to review your history before your consultation. Please include any relevant medical records to be reviewed at that same time.
  6. Please bring all medications and supplements you are taking to your initial appointment.

Payment

  1. Payment is due at the time of the visit. Payment may be made by cash, check, Visa or Mastercard. A late fee is assessed for any payments that are not made within fourteen days of the appointment.
  2. Dr. Fisch does not participate in any insurance plans. At the time of your visit, you will receive an invoice that may be submitted to your insurance company. Many patients do receive reimbursement; this depends on your individual plan.
  3. You will be asked to provide a credit card for your file for appointments and testing that is ordered.

Testing

  1. There is no on site laboratory at Essential Health. You will be given a lab order form to take to your local laboratory for testing that needs to be done.
  2. Testing done at local labs is generally covered by insurance.
  3. Specialty lab testing, including stool, saliva, blood and urine testing is done with kits that come with pre-paid mailing enveloped. These will be reviewed with you prior to anything being ordered. Some of these tests are partially or completely covered by insurance, and Dr. Fisch will discuss this with you at your appointment.
  4. There is a 20% cancellation fee for any testing canceled after it has been ordered.
  5. Test results are reviewed at the time of your next appointment. A copy of lab results will be provided at the time of the appointment.

General

  1. Medical records- There is a charge of 35 dollars for copies of medical records to be released to another office. Dr. Fisch does routinely provide a copy of all labs as well as a summary at the end of each appointment, and often this is sufficient to fill the need for medical records. If records can be sent via email, the fee is waived.
  2. Insurance- Dr. Fisch does not bill insurance. If there is a need for her to contact an insurance company, a fee is assessed, depending on the length of time needed to resolve the matter. Invoices are provided at the time of the appointment, and there is a fee for additional copies of invoices that are needed at a later date.
  3. Letters- if a letter is needed from Dr. Fisch for any reason, a fee will be assessed based on the complexity of the issue and the time it takes to complete the letter.
  4. Medication refills- you may request refills by email or phone. Please plan ahead and allow at least 2-3 business days for your request to be completed, as refills are done only on Mondays, Tuesdays and Fridays.
  5. Vitamins/supplements- Dr. Fisch does not sell vitamins on site, however, she will give you instructions how to purchase the vitamins she recommends at the appointment.
  6. PLEASE DO NOT WEAR SCENTED BODY PRODUCTS TO APPOINTMENTS, AS IT CAN BE VERY DANGEROUS FOR CHEMICALLY SENSITIVE PATIENTS WHO VISIT THE OFFICE AFTERWARD.
